In this Privacy Policy, 'We' or, 'Us' means Community Health Support Limited (ABN 93 656 141 936).

We are committed to respecting your privacy and are bound by the Australian Privacy Principles set out in the Privacy Act 1988 (Cth) ("Privacy Act").

By providing us with your personal information, you allow us to collect, use and disclosure of your information in accordance with this Privacy Policy.

What personal information do we collect and how do we collect it?

Personal information is any information which identifies and includes sensitive information such as health information.

We will usually collect the following types of personal information about you:

  • age or date of birth;
  • contact details (e.g.your address, telephone number, email address);
  • your location (for users of our app);
  • health information including your medical history, current conditions/illnesses and treatment provided by us to you; and
  • any other personal information that we might require to provide our services to you.

We may collect personal information either directly from you, or from third parties in any of the following ways:

  • by you registering or otherwise engaging with us via our website or app;
  • communicating with us through correspondence, chats, email, phone, or when you share information withus from other social applications, services or websites;
  • interaction with oursites, services, content and advertising; and
  • when this information is provided to us when you seek medical treatment or advice from us.

When you apply for a job or position with us we may collect information from you third parties such as from recruitment consultants, previous employers and others persons who might be able to provide information to assist us to decide whether or not to make you an offer of employment or engage with you under a contract.

Why and how do we collect, use and disclose your personal information?

We may collect, hold, use and disclose your personal information for any of the following purposes:

  • to enable us to provide you with our services and to respond to any requests for information that you might make of us;

 We may also use de-identified information about you for other purposes such as to provide training to our staff and reporting on our services.

You can access the personal information we hold about you by contacting us using the below contact details. Sometimes, we may not be able to provide you with access to all of your personal information and, where this is the case, we will tell you why.

Making a complaint

Please include your name, email address and/or telephone number and clearly describe your complaint. We will acknowledge your complaint and respond to you regarding your complaint within a reasonable period of time. If you think that we have failed to resolve the complaint satisfactorily, we will provide you with information about the further steps you can take.
